Médunik Canada is proud to offer a Patient Support Program for patients using Increlex^® and their caregivers. The Unik Access^® Program provides individualized and confidential support.

The highlights of this program are as follows:

• Reimbursement navigation
  • Assistance in drug coverage applications and coordination of benefits
  • Financial assistance (if required)
• Specialty pharmacy services
  • Patient counselling
  • Personalized distribution channel
• Patient follow-ups conducted by a Registered Nurse case manager
  • Education: At-home injection training with you and your child
  • Adherence and compliance

Your physician can easily enroll you in the Unik Access^® Program by completing the enrolment form and by forwarding it by fax at 1-877-670-7067 or e-mail at increlex@bayshore.ca . We will make all required follow-ups with you and/or your health care professional.
